It has been three months since the doomsday cult unleashed the genetically engineered Berlin C virus, and today most of the world is dead or dying. Among the living are a group of hospital patients and their nurse, Marie Alexander (Maria Conchita Alonso), who have survived because they were already under quarantine when the virus struck. They are running out of food and fuel when a soldier arrives with a new vaccine from the Center for Disease Control. But, there's only enough for three doses and it will take three days for the vaccine culture to develop enough to be effective. It falls to Marie to maintain order until the vaccine is ready, and to decide who will get injected.

Pembleton and Bayliss have only one lead to a potential suspect, a pager number; however, Bayliss has a plan that should lead to the apprehension of their suspect. Kellerman and Ballard get a case with a drug dealer that Kellerman is less than enthusiastic to work. The victim is one of Georgia Rae Mahoney's lieutenants and there are four other unsolved murders in the last two weeks where the victims were members of the Mahoney organization. Pembleton and Bayliss apprehend the wrong suspect, but the fact the suspect was trafficking 200 Kg of narcotics gets them a commendation. Meanwhile, using the information provided to him by Falsone, Lewis has had members of the Mahoney organization under surveillance. Is Lewis involved in these murders? C.C. offers herself to marry Maxwell, but his love for Fran is beyond money. Then Sheffield's lawyer arrives with the news: Concepcion decided to share her money, all because Fran made her feel like family. Niles plays tricks on Ms. Babcock, making her doubt her own sanity.

Janet has a crush on Steve, but he doesn't seem to get it. Donna discovers that the diamond jewelry that Noah gave her is fake.

Meanwhile, Jane has gone off to a Star Trek convention (""I like middle-aged men who are virgins""), leaving her pet Mr. Boots with Dharma and Greg; but Greg freaks out when he discovers that Mr. Boots is not exactly a cat.
